Participant Waiver & Release

I, the undersigned parent/guardian (or adult participant), hereby acknowledge that I have voluntarily enrolled the above-named participant in programs and activities offered by Mighty Mo Association ("MMA"), including but not limited to basketball training, skills camps, group sessions, and special events held in North Florida.

I understand and acknowledge that participation in athletic activities involves inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, and other physical injuries. I voluntarily assume all such risks on behalf of the participant.

In consideration of the participant being allowed to participate in MMA programs, I hereby release, waive, discharge, and covenant not to sue Mighty Mo Association, its directors, coaches, staff, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by the participant during participation in any MMA program or activity.

I consent to emergency medical treatment for the participant should it be deemed necessary by medical personnel. I agree to be financially responsible for any medical expenses incurred.

I grant Mighty Mo Association permission to use photographs or video of the participant taken during program activities for promotional and educational purposes, including social media and the MMA website, without compensation.

I certify that the participant is in good physical health and has no medical conditions that would prevent safe participation in athletic activities. I agree to notify MMA staff of any health conditions, allergies, or physical limitations prior to participation.

By signing below, I confirm that I have read this waiver in its entirety, understand its contents, and agree to be bound by its terms.
